RELEASE OF LIABILITY

I hereby apply for registration as a student of TAIKO CENTER of the PACIFIC, in conjunction with Kendo Music, and/or Washington Middle School, and/or the Hawaii State Department of Education. I, on behalf of myself, my heirs, assignors, executors, and administrators, do hereby release, relieve and discharge the TAIKO CENTER of the PACIFIC, KENDO MUSIC, TAIKO ARTS CENTER, WASHINGTON MIDDLE SCHOOL, THE HAWAII STATE DEPARTMENT OF EDUCATION, the KENNY ENDO TAIKO ENSEMBLE, and the state of Hawaii and their respective officers, agents, officials, and all other participants in the activities of the aforementioned groups and from any and all liability claims, demands, actions, and causes of actions that may arise from any injury to my person and/or property during my participation in the activities of TAIKO CENTER of the PACIFIC. I freely and voluntarily submit this application to the TAIKO CENTER of the PACIFIC. I am aware that this is a waiver and a release of liability and I voluntarily agree to its terms.


PHOTOGRAPH AND VIDEO RELEASE FORM (1) — Permission is hereby granted to TAIKO CENTER of the PACIFIC, TAIKO ARTS CENTER and KENDO MUSIC to utilize photographs or videos containing my image for the purposes of promoting past, present, and future TAIKO CENTER of the PACIFIC, KENDO MUSIC, and/or KENNY ENDO TAIKO ENSEMBLE events and activities. This includes, but is not limited to, using my image and/or likeness for marketing, public education, and promotional activities in printed and online media, video, and television without further permission or clearance from me. I understand that there will be no financial or other remuneration for the use of my image and/or likeness, for initial or subsequent transmission or distribution, and that TAIKO CENTER of the PACIFIC, TAIKO ARTS CENTER, KENDO MUSIC, and/or the TAIKO ARTS CENTER are not responsible for any expense or liability incurred as a result of my photograph being taken.(required)
